New Patient Registration
Becoming a new patient is easy with our online registration system. Once you schedule your appointment, our patient care coordinator will instruct you on how to complete the patient registration prior to your appointment. However, if you prefer paper forms, or have issues with our online registration system, you can download, print and complete our new patient registration packet which can be downloaded via the link below. Please bring the completed forms along with your dental insurance card to your appointment.

Your first appointment will start with a limited diagnostic exam, including any necessary X-rays. We’ll work with your general dentist to determine if your tooth is a good candidate for endodontic therapy and we’ll discuss treatment options with you.
If we determine a root canal is the best course of treatment, we will walk you through the procedure and next steps. The procedure normally involves local anesthesia and may be completed in one or more visits depending on the treatment required.
If we determine your tooth is not suitable for root canal treatment, or the prognosis changes during the course of treatment, we’ll inform you as soon as possible. In most cases, after endodontic treatment, you’ll be able to return to your normal routine right away.
While non-surgical treatment is often all that’s needed to save a tooth with an injured pulp, some cases may require endodontic microsurgery.
After your root canal, you’ll likely need to return to your general dentist to complete the final restoration of your tooth. It’s important to complete this step as soon as possible to avoid complications like tooth fracture or premature loss of the tooth.
